Inserts and other shoe modifications are usually covered if they are on a shoe that is an integral part of a brace and if they are medically necessary for the proper functioning of the brace. Some insurance companies require that the policy have a contract benefit for orthotics that does not exclude foot orthotics in order for inserts to be covered. Along with the orthotic coverage the inserts may only be covered if it is used to treat a medical problem or deformity.
